Compliance Reporting

A toll-free, continuously managed, compliance hotline is available to receive anonymous concerns regarding compliance matters (1-855-387-4416). A non-anonymous email is also available: compliance@snoqualmiehospital.org. Any concerns received by the Hospital’s Compliance Officer via phone or email will be logged into the Hospital’s event management system as are events that are reported from staff members who have identified or are self-reporting deviations from policy or other risk events.

Public Records Requests

King County Public Hospital District No. 4 will make public records of the district available in compliance with the Washington Public Records Act (RCW 42.56) and any other applicable provisions of federal or state law. The district will acknowledge the request within five business days and either make requested records available for inspection, provide copies, or give a reasonable estimate of when the information may be available.
